Sleep Record Instructions

To get the most accurate data for your treatment, please follow these guidelines carefully:

  • Please fill out this sleep diary every morning about 30 minutes after getting up.

  • Guess the approximate times. Do not worry if your numbers are not exactly correct—we are looking for general patterns, not perfection.

  • Do not check the clock during the night to fill this out; wait until the morning.

Data Security Notice: To protect your privacy and comply with healthcare regulations, please do not email completed sleep logs. If you prefer to submit your logs electronically before our session, please scan or photograph the pages and upload them securely via the Patient Portal.
